This form is intended for use by GRADUATES or FORMER STUDENTS of Woburn Memorial High School. It should be used when a graduate or former student would like to have their transcript sent to a third party (official transcript) and/or themselves (unofficial transcript). Be sure to complete all the requested information for each recipient and then sign and date the form at the bottom. Please complete one form for EACH recipient. Additional questions can be directed to Jessica Conway. Employers looking for education verification please fax requests to 781-937-8216. Please be aware it can take 1-10 days to process a request.
